Physical Delivery
The Application Form should be legibly completed and enclosed in a sealed envelope marked “CL Synergy Limited – IPO” be sent by courier, post or hand delivered to the Registrars to the Offer or designated branches of DFCC listed in Annex V before 4.30 pm on the Offer Closing Date. (Earliest closing date is 7th April 2022)
Applications that are supported by Cheques, Bank Drafts and Bank Guarantees MUST be physically delivered.

Care must be taken to follow the instructions on the reverse of the Application Form. Applications that do not strictly conform to such instructions and additional conditions set out hereunder will be rejected.
A softcopy of the physical application form is available on www.acuity.lk, ipo.clsynergy.com or www.cse.lk Exact size copies of the Application form printed on a clean paper as will be permissible.
Acuity's web portal
The applicant can use the above online application web portal which is also accessible via www.acuity.lk.
Only Individual Resident Sri Lankans can apply via the web portal. This option is NOT permitted for applicants applying via margin trading accounts, power of attorney, joint applicants, non-resident applicants, foreign citizens, corporate bodies and unit trusts.
The applicant must comply with the instructions stated on the web portal when submitting the online application.
Applications submitted via the web portal MUST be supported by an electronic fund transfer confirmation or a clear image of the bank deposit slip. Scanned images of Cheques, Bank Drafts or Bank Guarantees will NOT be accepted via the web portal, hence MUST be physically delivered as stated above.
CSE Mobile App
Applicants, who register with the CSE Mobile App, may complete the Application Form through such Mobile App, follow the instructions set out in the said Mobile App, and forward their Applications as per the instructions. Submission of digital Application Forms through the CSE Mobile App is limited to both citizens of Sri Lanka who are resident in or outside Sri Lanka and are above 18 years of age and, Foreign citizens above 18 years of age (irrespective of whether they are resident in Sri Lanka or overseas) only.
Prior to making payment for the Shares, Applicants should login to the CSE Mobile App, obtain the separate bank account details dedicated for this purpose and follow instructions.
The CSE mobile application can be downloaded from the Apple App Store (For Apple IOS Users) or the Google Play Store (For Google Android Users).

Prior to making payments for the Shares, the Applicant should pay attention to the below instructions.

When making online fund transfers and direct deposits, Applicants MUST state the CDS account/NIC number in the beneficiary remarks/narration/reference or an equivalent filed to identify the Applicant. In cases where the attached payment confirmations does not carry CDS account/NIC number in the reference filed for identification, the application will be rejected.

Applicants who intend to submit applications physically or using Acuity’s web portal as stated in (a) and (b) above should use the bank account details published below.

Applicants who intend to apply via the CSE Mobile App should log into the CSE Mobile App and obtain the separate bank account details given on the App for this purpose. Applications that do not use the correct bank account number for online fund transfers will be rejected.

Online fund transfers should be done via a Licensed Commercial Bank approved by the Central Bank of Sri Lanka.
ONLY the Applicants who apply via (a) Physical delivery or (b) Acuity’s web portal, can make payments electronically or over the counter directly to the bank account given below

The Applicant should clearly state the CDS account/NIC number in the beneficiary narration/remarks/ reference field in case of an online payment or in the “Purpose and Source of Funds” section in the DFCC bank deposit slip in the case of an over-the-counter deposit. Cash or cheque deposits to the bank accounts is accepted.
Applicants who apply via CSE Mobile App, should log into the App prior to making payments, obtain the separate bank account details dedicated for this purpose and follow instructions.
